SWSPHN is undertaking stakeholder and community consultations for the Care Finder program. We are inviting community members, health and aged care providers to attend one of our ‘Care Finder’ Local Health Forums.
What is the Care Finder program?
The Care Finder program is a Commonwealth funded aged care initiative. It is part of the response to the Royal Commission into Aged Care's recommendation for more localised and face-to-face supports to help people navigate access to aged care services. To complement the existing My Aged Care single entry portal, the PHN is aiming to fund Care Finder organisations. These organisations will help older people with intensive assistance to navigate the My Aged Care system and access aged care service supports in the community. Service delivery is expected from Jan 2023.
How can you be involved?
SWSPHN is undertaking stakeholder and community consultations for the Care Finder program. We are inviting community members, health and aged care providers to attend one of our ‘Local Health Forums - Care Finder Consultations’ (see Key Dates section on the right for details on the local health forum in your area).
Along with a presentation from South Western Sydney PHN CEO Dr Keith McDonald on our new Strategic Plan and some current activities, these Local Health Forums will involve a short presentation about what the care finder program is, how it is going to change the aged care sector and what information is needed to help guide the development of these services. The rest of the evening will be focused on gathering the insights and perspectives of health forum participants.
Meals will be provided during the health forum for all participants.
If you can't attend a face to face forum, you are encouraged to fill in the survey on this consultation page (below) to help us understand how local residents navigate aged care services and how we can improve this.
