    Your participation in this survey is voluntary and you are free to change your mind and withdraw from participating at any point before you submit your completed survey. Your feedback will remain anonymous. No information will be collected that will identify you personally.

    The survey should take you no more than 10 minutes depending on your responses. The information received will be used to help us understand how you use the antenatal HealthPathways specifically, and your preferences about accessing antenatal care information in general.
