We are South Western Sydney Primary Health Network (SWSPHN) and our role is to help improve the health of people living and working in South Western Sydney.

We want to hear from you about your health and your experiences accessing local healthcare services. This survey will take you about 10 minutes to complete. Your responses are anonymous which means that no one will be able to identify you.

Register your personal details and you may occasionally be invited to give confidential feedback on topics of interest to you, assisting with planning for improved health services in South Western Sydney.

There are 3 parts to this survey:

Part 1 – Register: Register your details before you take the survey if you want to go in the draw to WIN a $60 gift card

Part 2 – About you: This is to ensure we are hearing from a range of people living in our region

Part 3 – The survey: Your answers will help us understand your experience


After you complete the survey, we encourage you to share it with your friends, family and colleagues in our region.

How we will keep in touch

It is important to us that we share our findings with you as we value your input. We will post results to this project page once evaluation has taken place.

If you choose to register, you will have the option to select your preferred health topics during the registration process and will likely only be invited to contribute to other consultations and surveys a few times a year.

  • Health literacy relates to how people access, understand and use health information in ways that benefit their health. 

    People with high health literacy People with low health literacy are at higher risk of worse health outcomes and poorer health behaviours.
