  • Case Conferencing

    Diabetes case conferencing is available in south western Sydney. Specialist medical and allied health from South Western Sydney Local Health District are able to meet with you (in person at your practice or via telehealth) to develop a case management plan to support your patients with diabetes.

    Eligible patients are: Patients with type 2 diabetes with:

    • HbA1c ≥ 9% (8 to 8.9% included once those ≥ 9% have been discussed)
    • Significant and frequent hypoglycaemia
    • Recent hospitalisation, CVD or foot event in past 12 months
    • Nephropathy / raised ACR
    • Blood pressure over 160/100 mmHg
    • Triglycerides 10 + mmol/L
    • Women planning/at risk of pregnancy
    • Other (other patients with diabetes requiring discussion)

    MBS items are available to reimburse you for your time during the case conference.

  • Western Diabetes Education Program

    The Western Diabetes Education Program has replaced AUSCDEP is available for all SWSPHN clinicians for FREE. WDEP has been developed to improve the knowledge of clinicians working with people with diabetes.

    WDEP topics:

    • Promoting self-care (1 CPD hour)
    • What is diabetes? (2 CPD hours)
    • Hypoglycaemia (3 CPD hours)
    • Oral therapies (3 CPD hours)
    • Injectable therapies (3 CPD hours)
    • Nutrition (2 CPD hours)
    • Pregnancy: pre-conception care (1 CPD hour)
    • Hospital Hypos (1 CPD hour)
    • Safe use of insulin in hospital (1 CPD hour)
