Healthy Ageing at Home initiative in South Western Sydney

Some older Australians are entering aged care earlier than they need to. SWSPHN is aiming to fund services to help older people to remain health at home. This includes older people not currently receiving aged care services.
We have commissioned services to
- support early intervention programs that promote healthy ageing and management of chronic conditions.
- increase GP awareness of the needs of older people and available services.
To help us know where to focus our efforts, we sought input from the older residents and service providers of South Western Sydney. During 2022 we held a number of forums to better understand the needs of older people and, what should be prioritised to support healthy ageing at home. Our Healthy Ageing at Home project tender was finalised in October and Grand Pacific Health were the successful bidder. Well done!
The Healthy Ageing at Home service model provides an overview of the plan. If you live in Fairfield or Liverpool and are an older person with chronic disease, or a family or carer of an older person please get in touch.
We are also wanting to meet up with any community organisation providing support, services and care to older people, we want to help build capacity in our communities to enable older people to remain healthy at home.
Funded by the SWSPHN, Grand Pacific Health will be conducting community forums in Fairfield and Liverpool over the coming months, to ensure the services are useful to you please come along and let them know what you need.
News Update!
Co design workshops with Arabic and Vietnamese older people, their family and carers and community agencies has been completed for 2023. A summary report is now available which will inform the project team on the educational workshop themes. Significantly, emotional health, dealing with trauma, navigating Aged Care supports in their community and how to better access the Commonwealth Home Support Service coupled with early interventions to manage chronic disease will feature in the educational workshop series to be commenced later this year.
